CARE PLANNING / IMPLEMENTATION AND EVALUATION IN ADPIE

 

The Nursing Process

A nurse who is new to the clinical setting, knows all
too well it is a challenge. Typically, a combination of
learning in the classroom and a clinical area is done.

However, feeling confident and demonstrating competence
takes time and practice.

PLANNING ( IN ADPIE )

Mrs. G was admitted to the hospital with complaints
of headaches, visual disturbances and occasional
unsteady gait. This has been going on for about two
weeks. 

Her family members decided to seek medical advice.
The doctor has examined her and ordered a CT scan
of the head.

The nurse who is responsible for her care, begins to work
on the care plan. She will identify the problems based
on the diagnosis. Other staff members will also assist
in the process.

IMPLEMENTATION ( IN ADPIE)

The nurse prepares Mrs.G for a CAT scan. She collects
information regarding allergies, as contrast dye may be used.
She also explains to the family what is going to be done. 

If Mrs. G requires surgical intervention, this will be reflected 
in the care plan. Following surgery, she will be recovered in 
PACU, then sent to a surgical floor or ICU.

EVALUATION ( IN ADPIE

The nurse is responsible for evaluation and documentation
of patient progress based on the care plan. The care plan is
usually updated daily and as any changes occur.

General anesthetics are usually given prior to surgery.

What kinds of drugs are used as anesthetics ?
IV drugs such as Opiates(Fentanyl), Benzodiazepines (Midazolam),
Barbiturates (Thiopental), Hypnotics (Propofol), are some examples
of the medications that are used to put patients to sleep.

It is important to recognize that a patient who has received 
anesthetics in surgery, have the potential for a paralytic ileus.
This and other potential surgical problems, should be reflected 
in the care plan.
