TRAUMA/SEPSIS CASE STUDY CONTINUES

Nick's progress in the ICU.

Nick is currently in the ICU. The day following
admission:

Postoperative day 1 - extubation ( his endotracheal
tube was removed )
was done.Prior to removal of the
endotracheal tube, an ABG was done. Results
can be seen above. Supplemental oxygen via

Postoperative days 2 and 3:

Nick continues to make excellent progress.
He is afebrile. His lab values are within normal
limits. He also has a nasogastric tube which is
connected to wall suction.

Why was TPN ordered for Nick?
Total Parenteral Nutrition (TPN) was ordered
by the doctor, as Nick would not be able to eat
and drink as normal. Adequate nutrition would
be provided via TPN. A central line and special
IV tubing with a filter, is used.

What is septic shock?

The image above shows Nick's progress from
Postoperative days: 4 -7.

Postoperative day 4 ( Possible Septic event).
The nurse does an assessment and documents Nick
has a fever.The doctor was notified and lab values 
were ordered. 

Nick/s abdominal incision was intact, but the skin
is reddened and tender. Antibiotics were ordered
and Nick's fever finally subsided.

Postoperative days 5-7

Nick is afebrile . His nasogastric tube is removed
a clear liquid diet is ordered. Physical therapy will
start soon.

Postoperative days 8 - 9

Nick's incision is clean and intact. The tension 
sutures are removed. He has been started on a
clear liquid diet,which was well tolerated. 

Nick has now progressed to a full liquid diet.
 The physical therapist is at his bedside to
begin exercises.
